Application Information

Applications for a sabbatical fellowship award should be submitted online by October 1, 2008. Please note that no applications will be accepted after the deadline date. Identification of the successful applicants will be made no later than December 15, 2008.

Applicants must provide the following information:

1. Registration Information. Submit your information online by following the steps in the registration process.

2. An Extended Letter containing the application narrative that must include the following information.

(a) Applicant
Name, Citizenship/Permanent Residency, Curriculum Vitae, Current Sources of Program Support, Past Mentors, Collaborators, Trainees Mentored, Bibliography, and Future Aims

(b) Sponsor
Copy of sponsor/host/mentor curriculum vitae, including education and training, experience in research, past and current research support, and bibliography submitted in NIH format/PHS 398. Please limit this to no more than three pages.

In addition, a letter should include a brief description of the training and research facilities that will be available to the candidate at the host institution, and an indication of approval of the applicant's plan from all collaborators participating in the sabbatical activity.

(c) Position
Department(s) in which the sabbatical activities will be established and the nature of collaborative agreements with other departments participating in the education and training activities. This includes identification of other individuals with whom the applicant will be directly involved and in what fashion. Date on which the position described will be activated.

(d) Plan
The applicant must prepare a comprehensive statement, not to exceed eight, single-spaced, typewritten pages of 12 point or larger, of the plans for research, and for education and training in Health Outcomes. Applicants are requested to use a format similar to that specified for the submission of grants to the National Institutes of Health, which includes a description of the objective(s), rationale, methods of approach and a bibliography of major references. Preliminary results should be included in the narrative of the research proposal and preprints or reprints of not more than three key papers may be included.

3. References. Two letters of recommendation from those closely associated with the candidate during his/her scientific career are required. These should be sent directly to the Foundation. When possible, these letters should be from individuals not in the institution sponsoring the candidate.

Also, a letter from the Chair of the applicant's home department confirming approval for the sabbatical leave must accompany the application.

4. Copies (PDF or word doc.) or appropriate web link of each article published or ready for publication by the candidate that have a bearing on the intended research or the individual's prior accomplishments should accompany the application. Scanned photocopies of articles furnished to the PhRMA Foundation in fulfillment of this requirement must be in compliance with U.S. copyright law.

5. Please note if application involves research on drugs for rare diseases.

During the review process, we will be considering the following information:
